When playing guitar, in songs, there are some that do an open chord and then switch to a barre chord right after... For instance, I'm trying out playing D and then going to Bm, but I find I'm coming in late on the Bm when I play along with the song I'm practicing, trying to place my fingers again after the D...

Are there any things I can do to help myself get faster when switching from open chords to barre chords, like exercises or something that you know of? Or is it just more of the motto "practice practice practice"? :)

I'd say it's just "practice, practice, practice". As with all new techniques on guitar, the best way to get better is to slow down, get used to the chord shapes, and take you time.
